To form chords using this table, go to the appropriate key and pick out the notes indicated. For example, for an E minor (Em) chord, the table indicates the notes 1 - b3 - 5. So, from Table 2, find the key of E and examine the notes 1 - 3 - 5. They are E, F#, G#, and B. Use the first note (E), the third note flatted (F), and the fifth note (B). These three notes form Em. |

To form Dmin9+ (D minor 9th augmented), the table indicates (under Augmented Minor 9th) the notes 1 - b3 - #5 - b7 - 9. So we use Table 2, find the key of D, and see the notes 1 - 3 - 5 - 7 - 9 are D, F#, A, C#, and E. We then flat the third and the seventh, and sharp the fifth, to get the notes D, F, A#, C, and E. These are the notes we use to form the chord.
