On January 28, 1986, shortly after launch, the Space Shuttle Challenger blew up. Subsequent investigation determined that NASA had a flawed safety program — and had, in fact, deferred safety in order to increase the number of launches.
Richard Feynman's synopsis, Appendix F (Feynmann was the Nobel-Prize winning physicist who served on the investigating commission), is one of the best summaries of the problems leading up to the accident. It appears in several places on the web. I have also put a formatted copy of the report on this site. There are several other Feynman links on the web. |
